A good mental health assessment can be key to ensuring the right next steps for someone on their mental health journey.

If you enjoy being part of a fast-paced team that is committed to finding solutions for those people with complex and severe mental health needs - then this is the role for you.

We welcome applicants with a wide range of mental health experiences and professional registration including RMNs, RNLD, OTs or social workers, who are able to work independently to provide holistic mental health assessments. You will understand what it takes to be the first professional to listen to someone’s story and to use this along with your clinical knowledge to signpost the next best step across a range of NHS, social care and VCSE services. You will be a team player used to juggling a busy diary. You will be a good communicator, building positive working relationships and connecting with a range of health and social care professionals.

We are a diverse team of mental health nurses, occupational therapists, social workers and administrators. We have recently welcomed a number of mental health and wellbeing practitioners into the team, meaning we also offer brief interventions within the team.

We are currently seeking clinicians to work in the Mid and West ICA areas  of the county of Cornwall

### Main duties of the job

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

You will be working with your colleagues to manage the referrals coming through to secondary mental health services via triage and initial assessment.

You will be expected to work in partnership working with GPs, other referring agencies and community mental health teams including, Early Interventions in Psychosis, Home Treatment Team and Talking Therapies.

The assessment service is currently developing and is in a period of transformation, so now is an exciting time to be joining and involved with the implementation and development of news ways of working.

## Person Specification

### Experience

**Essential**

- Registered mental health professional with substantial post registration experience.
- Previous experience working in a community setting

### Education and Qualifications

**Essential**

- Relevant professional registration – RMN, OT, SW, RNLD
- Evidence of post basic training
- Current professional registration
- To have a Sign Off Mentor (SOM) qualification as in accordance with the NMC and be able to sign off students as proficient of registration.
