### Job overview

We are currently looking for a part time Band 6 Memory Nurse in the East/North East locality for 28.5 hours a week, working days are negotiable over Monday-Friday. This locality is based near Chapeltown in Leeds LS7 and covers the East/ North East part of the city. We are looking for a committed and motivated practitioner who wants to consolidate their nursing practice in memory services and develop their skills, experience and knowledge. The successful applicant will be expected to demonstrate the capacity to work independently in the assessment for dementia process and offering post diagnostic support.

It is desirable for the candidate to have post registration experience, and experience with memory assessments and evidence based tools used for diagnosis. You will be able to demonstrate an in depth understanding of the needs of service users with dementia and have an ability to support service users and their families/carers practically and emotionally post diagnosis.

You will need to be able to work independently out in the community as well as being part of a Multi-Disciplinary team. The current role within the team does include hybrid working, as well as running a clinic once a week.

### Main duties of the job

- Undertake comprehensive assessments of individuals referred with memory concerns, including cognitive, mental health, physical health, and social care needs.
- Conduct clinical interviews and utilise recognised cognitive assessment tools to support diagnostic decision-making.
- Gather collateral information from carers, families, and other professionals to inform assessments.
- Contribute to the diagnostic process and present findings within multidisciplinary team meetings.
- Communicate assessment outcomes sensitively and effectively to patients and their families.
- Provide information, education, and emotional support to individuals living with dementia and their carers.
- Monitor the effectiveness and tolerability of anti-dementia medications and undertake medication reviews within scope of practice.
- Identify and manage risks associated with cognitive impairment, including safeguarding concerns, self-neglect, vulnerability, and carer stress.
- Liaise with primary care, secondary care, social services, voluntary organisations, and community teams to ensure coordinated care.
- Support patients through post-diagnostic interventions and signpost to appropriate community resources and support services.
- Maintain accurate, timely, and comprehensive clinical records in accordance with professional and organisational standards.

### Detailed job description and main responsibilities

The service operates between Monday and Friday, 9am till 5pm.

You will need to be able to work independently out in the community. It is essential that you have a full UK driving licence and use of a car. The successful applicant will be offered a comprehensive continuing professional development package which will include access to appraisal, clinical and managerial supervision.
