We are looking for part time and full time pharmacists to provide a clinical pharmacy service to our inpatients in our Rehabilitation Service across the Trust Are you looking for a rewarding career within the pharmacy world? With the aim of shifting health care from acute hospitals to settings closer to people’s homes, and moving away from reactive care to preventative care based on early interventions. ### Main duties of the job As an integral member of the team, you will be providing specialist expertise in the use of medicines whilst supporting to address both public health and social care needs of patients. You will be involved in conducting structured medication reviews for patients with polypharmacy, long term conditions or high risk patients ensuring an integrated approach to the management of medicines. If you are successful you will join the existing team of Band 7 pharmacists at the Trust covering a range of rehabilitation specialties. You will be supported by experienced Senior Clinical Pharmacists allowing you to gain new skills in each rotation and the ideal opportunity to develop strategic, quality improvement and leadership skills. To provide support and guidance to Medicine Management Pharmacy Technicians To be responsible for individualised patient pharmaceutical care ensuring clinical prioritisation on a day to day basis at ward level. To provide a patient focused pharmacy service to patients and staff including medicines reconciliation and full clinical screening of prescriptions to ensure safe and cost-effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimized e.g. NICE, NHSE/I, PHE Facilitate changes to the patients’ own use of medicines to improve safety, adherence and empower as much independent use of medicines as possible and appropriate.
