We are are looking for an enthusiastic medicines optimisation assistant to join our newly developed Urgent & Emergency Department Pharmacy Services at Bradford Teaching Hospitals.

This is an exciting opportunity to be part of a developing service by supporting patients within the Emergency Department and Acute Assessment areas.  The succesful candidate will play a key role in supporting patient flow across the hospital, ensuring the supply of emergency and time-critical medications as well as improving stock management within the department.

The role presents a chance to work within a multi-disciplinary team delivering fast-paced and high-impact care for some of the most vulnerable patients within our vibrant community.

The ideal candidate will be a confident communicator, an effective team member and adaptable to new challenges.

### Main duties of the job

Supporting the supply and dispensing of urgent and time-critical medicines for patients.

Assisting in the identification of patients planned for discharge and faciliating the timely ordering of discharge medicines (TTO's)

Liaising with Pharmacy, nursing and medical staff to minimise delays in medicines supply and supporting patient flow

Communicating with patients and carers to establish availability of medicines at home or via community pharmacies

Maintaining appropriate ward stock levels, including stock rotation and exipry date checking and using own initiative to identify areas for improvement

Prioritsing workload effectively in a busy clinical environment with competing demands

Participating in ward handovers and contributing to effective multi-disciplinary team working

Urgent and Emergency Care services operate within a high-demand environment, managing patients with complex health needs and often requiring rapid assessment, treatment and discharge.  Pharmacy Services  play a key role in supporting safe and effective medicines use within these pathways.

- NVQ level 2 in Pharmacy Services* (including units 5 ‘Assemble Prescribed Items’ and unit 15 ‘Undertake an in-process accuracy check of assembled prescribed items prior to the final accuracy check’) or equivalent Grandparent Clauses *or in the process of working towards this qualification
