The successful candidate will provide a ward based clinical pharmacy technician service to our oncology and haematology patients, which will include working closely with the nurses to ensure a medicines management is maintained at a high standard on the wards and providing an individualised medication counselling service to patients. The postholder will work closely with the multi-disciplinary team and will have the opportunity to work across different clinical settings including inpatient wards, daycare, outpatients, and ambulatory care. To be successful in this role, you will need to be organised, motivated, have excellent communication and people skills as well as the ability to adapt quickly to new situations. Registration as a pharmacy technician with the General Pharmaceutical Council (GPhC) is essential as well as holding ACPT accreditation or be willing to work towards it. Previous experience of medicines management at ward level would be a significant advantage. In return we can offer a professionally rewarding career with the opportunity for personal development along the way. ### Main duties of the job The main day to day responsibilities would include: - Liaise with pharmacy, nursing, medical and other ward staff in order to facilitate the medicines management process for Cancer Services. - To facilitate the dispensing process for Cancer Services based in the Macmillan Cancer Centre. - To ensure timely medicines reconciliation is carried out for all in-patients. - To review patient’s own drugs and assess their suitability for re-use. - To cross check with patients if their medication history sent by their GP is up to date and correct and to check concordance with the patient. - To inform patients about the medicines management scheme and what to expect during their stay. - To counsel patients regarding the safe and appropriate use of their medicines. - To participate in ward multi-disciplinary team meetings and to liaise with ward staff on medicine management issues. ### Detailed job description and main responsibilities For the full Person Specification and more information regarding the main responsibilities of this role, please refer to the attached Job Description. The pharmacy cancer team is well integrated into the cancer directorate and provides a high-quality and progressive pharmacy service to cancer patients.
