Are you our NEXT Type 1 Diabetes Multi-Disciplinary Team (T1DE MDT) hero?

Could you help us provide great patient friendly service and support? Are you looking for an exciting challenge working as part of a busy team

If so, you could start off your career supporting the T1DE MDT. T1DE is a funded project specially created for patients with Type 1 diabetes disordered eating and is currently funded to the end of November 2026.

This exciting post will allow the successful candidate to collate and distribute the MDT patients for discussion, ensuring that all relevant information is available for discussion at the MDT, recording outcomes and then next steps accurately and updating RTT accordingly.

The post holder will be expected to contribute to the provision of the highest standards.

### Main duties of the job

You will be required to:

- provide high quality support to the department.
- primarily support Diabetes Consultants, nurses and phycologists from NNUH in the preparation of the MDT and then following up the outcomes and booking patients accordingly.
- have excellent communication skills.
- handle patient information on a daily basis, so you will be expected to have a high level of accuracy and confidentiality.
- need good data entry and IT skills.
- prioritise your workload and work on the departmental database systems.

Being a good team player is a must, and you will uphold the Trust’s PRIDE values.

Interviews to be arranged for 24/06/2026.

### Detailed job description and main responsibilities

- Arrange the weekly MDT meeting; ensure appropriate venue and equipment available.
- Collate, produce and disseminate the MDT list of patients for discussion in collaboration with the Clinical Team to include all appropriate new and historic cases.
- Ensure all information to enable a complete discussion is available reporting any discrepancies to the MDT lead.
- Ensure discussion is recorded and disseminated appropriately including communicating information to GP’s.
- In collaboration with the clinical team ensure all actions resulting from the MDT discussion are implemented.
- Maintain an accurate record of team attendance at MDT, producing a yearly report for the MDT lead.
- To be aware of availability of staff and events which may impact on the MDT meetings ( eg consultant leave, MDT meeting cancelled due to bank holidays) and liaise with the Patient Pathway Coordinator to make allowances for this in the patient pathways.
- Develop systems to ensure rapid and effective communication between all members of the MDT.
- Collate / Input clinical data from MDT into the Somerset Cancer Registry under the direction of the MDT lead.
