### Job summary

As a Matron for our Community Teams, you will lead and manage our teams to ensure a collaborated approach to patient care focussing on quality, experience and patient outcomes.

We are looking for a motivated and innovative leader, with a passion for quality patient care and a drive for a position of continuous improvement.

The NHS 10-year plan emphasises patient-centred care with a focus on the shift from hospital to community care. Highlighting the importance of multidisciplinary collaboration to develop and implement care plans that meet the needs of patients closer to home. The plan's focus on prevention and early intervention will require you to ensure compliance with policies and guidelines that support preventive care, health education, and early intervention programs to improve population health. ### Main duties of the job

Your responsibilities will include overseeing the delivery of community services, ensuring the implementation of best practices, and fostering a culture of continuous improvement and patient-centred care.
