An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Physiotherapist to join our Brent Community Rehabilitation Team for a 6-month fixed-term contract, based at Wembley Centre for Health and Care.

Please note: Due to the fixed-term nature of this post, it is not eligible for Skilled Worker visa sponsorship.

As part of our friendly and supportive multidisciplinary team, you will deliver high-quality, patient-centred rehabilitation within the community. You will manage a varied caseload of patients, supporting individuals to maximise their independence and achieve their rehabilitation goals in their own homes.

The team forms part of the wider Integrated Community Falls and Neuro Rehabilitation Service and works collaboratively alongside District Nursing and Integrated Care Coordination Services, providing truly integrated care across Brent.

We are looking for a physiotherapist with experience in rehabilitation, community services and/or older adult care who enjoys working autonomously while collaborating closely with the wider multidisciplinary team. The role includes participation in a 7-day rota.

### Main duties of the job

- Undertake comprehensive specialist physiotherapy assessments independently and as part of a multidisciplinary team within patients' homes and community settings.
- Assess, diagnose and manage a diverse caseload of patients presenting with a range of complex conditions, including musculoskeletal, neurological and frailty-related conditions.
- Develop, implement and evaluate evidence-based, person-centred treatment and rehabilitation plans, adapting interventions to meet each individual's clinical needs and rehabilitation goals.
- Work collaboratively with multidisciplinary colleagues, carers and external agencies to promote safe, effective and coordinated care that supports patients to maximise their independence and remain within their own homes wherever possible.

## Person Specification

### Experience

**Essential**

- Experience post registration as a Band 5 physiotherapist
- Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory

**Desirable**

- Experience working with voluntary organisation
- Experience in service development projects

### Key Attributes

**Desirable**

- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

### Skills and Knowledge

**Essential**

- Skills in the assessment and treatment of adults with complex multi-pathologies and Knowledge and understanding of complex medical conditions including neurological conditions, multipathologies and falls and neurological conditions
- Knowledge of role of Allied Health Professionals and interdisciplinary team working
- Ability to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting, knowledge of outcome measures and evidence-based practice & ability to problem solve, prioritise and demonstrate clinical reasoning
- Good communication and report writing skills, in both verbal and written format
- Teaching of junior staff / students, peers in Inservice training setting & Involvement in Appraisal Scheme
- Knowledge and understanding of the role of Social Services and voluntary agencies
- Basic IT skills – Microsoft Word, Power point, Excel

**Desirable**

- Supervision of assistant / junior staff / Students
- Knowledge and experience in research and audit ,
- Knowledge and experience in research and audit
- Knowledge and understanding of National Service Framework for Older People
- Knowledge and understanding of relevant NICE and CSP/COT Guidelines

### Education/Qualification

**Essential**

- Diploma/degree in Physiotherapy
- Current HCPC registration
- Evidence of up to date CPD portfolio , including attendance at specialist short courses or equivalent CPD activities within the relevant areas of Neurology,

**Desirable**

- Member of a Special Interest Group relevant to this post
