Working independently and alongside colleagues, you'll travel throughout the community, providing personalised care tailored to each individual's needs and circumstances. Your kindness, professionalism and ability to build meaningful connections will have a lasting impact on those we support. As a Healthcare Assistant, you'll deliver hands-on personal care, assist with medication, support daily living activities and, where required, help with meal preparation. You'll be part of a fast-paced service supporting people recently discharged from hospital, those with palliative care needs, and individuals approaching the end of life. Working closely with District Nurses, Health and Social Care Partnership teams, hospice colleagues and wider multidisciplinary professionals, you'll play a key role in ensuring seamless, compassionate care. While much of your work will be supporting patients in their own homes, there may also be occasions where you'll provide support to teams within the hospice environment.
