### Main duties of the job

The successful post holder will be a named key worker with the expectation to work collaboratively with others to develop and use appropriate strategies and opportunities to share knowledge with, deliver interventions and influence the practice of colleagues within the UGI service.

The role of the Macmillan UGI Clinical Nurse Specialist is to be a dedicated key worker to adult patients diagnosed with a Gastrointestinal cancer, coordinating care between secondary and primary care, and between secondary and tertiary centres and providing continuity over time.

The post holder will be responsible for the contributing to the on-going development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues.

### Detailed job description and main responsibilities

The Macmillan UGI CNS will be a member of the Gastrointestinal Medway Service working closely with the UGI consultants and Medical Oncology Consultants, along with the Macmillan CNS team to support individuals diagnosed with UGI cancer.

To actively participate as an integral part of a multidisciplinary team; attend MDT meetings and weekly ward rounds.  In conjunction with the MDT and support services, the post holder will assist with the co-ordination of the treatment pathway, incorporating information provision, physical and psychological support and completion of holistic needs assessments.

The post holder will provide specialist advice and support to provide co-ordinated high quality patient centred care for patients with UGI cancer during the cancer patient journey from diagnosis, treatment, follow up and palliative care both as an inpatient, outpatient and liaise with healthcare providers within the community setting,

To practice autonomously, applying specialist expertise and knowledge of UGI cancer in providing patient-centred care during cancer patient journey (investigations, diagnosis, treatment, follow-up,  palliative care)

To be wholly accountable for own clinical practice; continually strive to expand professional expertise, knowledge and evidence-based practice by availing self of educational opportunities (seminars, conferences, tertiary courses); actively participate in professional organisations

To ensure that specialist counselling and health promotion are integrated into programmes of care and liaison is facilitated within MDT

To act as advocate for patients and respect diverse cultural backgrounds and needs

To offer a supportive service including signposting for counselling to patients and their family/friends or carers through their cancer patient journey in conjunction with other healthcare professionals, empowering patients to make informed choices and decisions.

To maintain Nurse-led clinic's whereby the concerns and needs of the patient's and their family/friends and carers are appropriately channelled

### Skills

**Essential**

- Good communication skills and ability to discuss sensitive information. Proven listening / counselling skills with the ability to manage distressing and complex situations
- Ability to assess, plan, implement and evaluate specialist care delivered to patients and give relevant information and advise to patients and relatives
- Excellent verbal, written, interpersonal and communication skills

### Knowledge

**Essential**

- Ability to clinically lead and influence staff
- Up to date knowledge of current clinical and professional issues
- Sound knowledge of current issues in the profession and practice of nursing
- Demonstrate an awareness of clinical governance and risk management and the escalation process

### Experience

**Essential**

- Experience of delivering patient advocacy and management of own caseload
- Experience of audit, research and evidence based care
- Experience of delivering change management both personally and as a facilitator
- Experience and competent in the management of patients within the speciality

**Desirable**

### Qualifications

**Essential**

- Registered Nurse (Level 1) on the NMC Register. To hold the appropriate level for the area of speciality
- BSc Nursing or equivalent qualification/training and experience
