An exciting opportunity has arisen for a Macmillan End of Life Care Facilitator to join the Specialist Palliative Care Team within Powys Teaching Board. We are seeking a compassionate and dedicated individual. The ideal candidate will possess excellent communication skills, a deep sense of empathy, and experience in palliative care.

The post holder will work within national and local End of Life guidance, ensuring best practice in End-of-Life care for all conditions and across Health and Social Care settings.

The post holder will develop and facilitate an end-of-life education programme for professionals in Powys Teaching Health Board to ensure staff have the right skills and knowledge to deliver the best end of life care.

The post holder will work with the Powys Macmillan End of Life GP facilitators to support teams with Advance Care Planning to ensure that people are enabled to make plans and choices about their care.

We would particularly welcome applications from Welsh speakers; but even if you don’t speak any Welsh, or want to develop your skills, don’t worry: the health board will support you with training.

Play a key leadership role within the Health Board to promote and support best practice in end-of-life care.

Interpret national priorities into local plans for continuous improvement.

Working with the Macmillan GP End of Life Facilitators and the Specialist Palliative Care Team actively promote and support all teams with advance care planning to support people to be cared for in their preferred place of care.

Contribute to the development of plans and strategies for palliative care, based on national direction and clinical need. Develop learning resources to support staff with advance care planning.

Working with the nursing homes identify reasons for admission at end of life and pro-actively support the homes to make changes to address these.

Lead the development of patient pathways to support end of life care linking with a range of professionals internal and external to the organisation, maintaining a collaborative and integrated approach.

### Qualifications and/or Knowledge

**Essential**

- Live registration with the NMC as a Registered Nurse
- Post Registration specialist training / supplementary qualifications to Masters level in related subject or equivalent experience
- Teaching qualification/ Teaching and Assessing in Clinical Practice or evidence of teaching skills/ clinical educator
- High level of knowledge of NMC code of conduct and professional standards/guidance
- Knowledge of the end-of-life Quality Statement

**Desirable**

- MSc Advanced Clinical Practice or working towards
- Management/ Leadership Qualification or equivalent preparation
