You will be part of a dynamic team that strives to provide a responsive and high quality Specialist Palliative Care Service to our patients and Colleagues and in doing so support each other through team learning and supervision. The Specialist Palliative Care Team provides advice, information, education and support to adult patients and their families during the last year of life. The team provide a day to day specialist palliative care nursing resource across the Trust and are supported by an established team of senior nurses and medical clinicians. It also contributes to the delivery of End of Life Care education and training for Trust staff. The post holder will have a strong clinical role evaluating individual patient needs including assessment, history taking and treating patients.  This role has responsibilities across all clinical and educational sites within the Trust.
