We are seeking a passionate and motivated practitioner to join our Specialist Lymphoedema Service, playing a key role in delivering high-quality, patient-centred care across Norfolk. This is an exciting opportunity to work alongside experienced clinical leaders and a wider multidisciplinary skin integrity team, helping to shape and develop specialist lymphoedema services for the future.

As a valued member of the team, you will contribute to the delivery and ongoing development of specialist lymphoedema care, ensuring patients and their carers receive clinically effective, compassionate support tailored to their individual needs. You will have the opportunity to use and further develop your specialist knowledge while influencing both specialist and generalist practice across the healthcare community.

Supported by the Lymphoedema Team Lead and Clinical Lead, you will benefit from strong clinical leadership, supervision and opportunities for professional development. ### Main duties of the job

- Assess, plan, coordinate and deliver specialist lymphoedema care, ensuring personalised, evidence-based treatment and ongoing evaluation of patient outcomes across healthcare settings.
- Manage a caseload of patients, including receiving and triaging referrals, undertaking comprehensive assessments, developing treatment plans, and monitoring progress.
- Facilitate effective communication between patients, carers, healthcare professionals, and external agencies to ensure seamless care pathways.
- Advocate for patients, supporting informed decision-making and consent processes.
- Contribute to service development, implementation of policies, protocols, and quality improvement initiatives.
- Maintain professional competence through continuing professional development, clinical supervision, appraisal, and reflective practice.
- Provide leadership, support change management, and contribute to a safe, high-quality care environment.
- Mentor, train, and support students and staff, contributing to specialist education and workforce development.
- Plan and deliver specialist training programmes and act as a resource for lymphoedema knowledge across the organisation.
- Maintain accurate clinical records, collect service activity data, and utilise information systems to support patient care, audit, and service evaluation

**Essential**

- Some roles require additional level of checks as part of the employment process i.e. roles which come into contact with children, vulnerable adults or their families.
- Must hold full and valid driving licence and have access to a vehicle as will be required to work in satellite clinics in different areas of Norfolk as well as patient homes.
- Able to communicate effectively in written and verbal English Language

### Experience

**Essential**

- Experienced nurse/HCP with relevant post registration experience
- Experience of working within a clinic setting

**Desirable**

- Able to demonstrate ‘evidence’ of working within the MDT
- Experience of audit/research
- Experience of teaching in a clinical area
- Experience of working in a community setting

### Communication

**Essential**

- Excellent verbal and written communication skills
- Good interpersonal skills
- Able to work as a team member
- Comprehensive working knowledge of Microsoft Office, especially Word, Excel and Outlook

### Qualifications and Training

**Essential**

- Current NMC or HCP registration.
- 1st level degree or working towards Health professional qualification.
- Evidence of continued professional development

**Desirable**

- Qualification in the management and treatment of lymphoedema

- Physically able to perform the requirements of the role physical skills where accuracy is required e.g. bandaging of limbs
- Has the ability to evaluate own strengths and weaknesses
- Experienced of mentoring and developing staff.
