An exciting opportunity has arisen for a Long Term Condition Nurse at The Wells Road Centre.

The Wells Road Centre provides low secure inpatient services for adult men and women with mental illness and/or personality disorder, as well as services to adult men with intellectual disability.

This role requires a Registered General Nurse to work as part of a team delivering primary healthcare within a low secure setting.

The post holder will be required to perform some interventions on the mental health wards.

### Main duties of the job

The post holder will provide highly quality primary healthcare to low secure mental health patients, working as part of multidisciplinary as a Long-term Conditions nurse.

To lead of primary healthcare delivery, including;

- Vaccinations

- Women’s healthcare

- Annual physical healthcare checks

- Chronic, complex wound management

- Ear care, including hearing checks and micro suction of ear wax

- National Health screening programmes

- Long term condition management use the quality outcomes framework.

### Detailed job description and main responsibilities

Responsible for the assessment, planning, implementation and evaluation of healthcare for patients.

Safe administration of medication in accordance with National, Trust and Local policies

Provide a triage service and clinical interventions, recognising environmental limitations and the need for appropriate referral, assessment and transfer of patients who require care within wider NHS provision.

Respond to healthcare emergencies within the hospital in accordance with the Trust's and Local policies and procedures.

Maintain accurate healthcare records

Deliver evidence based care

Seek specialist advice from other Healthcare professionals both internal and external.

Handles sensitive and confidential information appropriately.

Organise and be responsible for the allocation of workload to deliver effective and efficient high quality care.

To provide basic leadership to junior staff by maintaining professional standards ensuring adherence to all relevant policies and procedures to ensure the delivery of high quality care.

Promote independent self-management of patients healthcare needs

Respect Patients individuality and maintain their privacy and dignity within the constraints of the High secure environment.

Actively participate in the development of the healthcare services within the secure hospital setting.

Ensures the effective and efficient use of resources within their own sphere of responsibility.

Prioritises own workload based on clinical priority

Responsible for identifying own learning needs and participate in training to develop and maintain own knowledge, personal qualities and skills.

To actively participate in all aspects of clinical and managerial supervision

Take responsibility for your own and others' health and safety in the working environment.

To work within the provision of and keep up to date with the Trust and Local policies and procedures

To undertake any other duties that would be a reasonable expectation of the role

Able to recognise and maintain appropriate professional boundaries at all times and report inappropriate behaviour/boundary concerns

Maintain own registration requirements in accordance professional body regulation

Provide mentorship and preceptorship to junior staff and learners

You must have appropriate UK professional registration.

### Skills

**Essential**

- Ability to work independently and prioritise own workload.
- Record keeping and report writing skills.
- Good communication, negotiation and liaison skills (both oral and written).
- IT skills including use of patient information systems, with ability to learn to effectively utilise service specific computer packages.
- Ability to function within a team, demonstration of leadership skills and cohesive working relationships.
- Suturing skills or willingness to undertake appropriate training
- Ability to supervise and act as a mentor to both registered and un-registered members of the team
- Competent physical health Triage skills

**Desirable**

- Appropriately trained in Minor Surgery skills or willingness to undertake training

### Knowledge

**Essential**

- Knowledge of political drivers e.g. NHS agenda, national service frameworks, service delivery against performance targets and their impact on Forensic Services
- Professional and clinical knowledge acquired through training
- Developing knowledge of physical and mental healthcare in secure forensic setting

### Experience

**Essential**

- Experience of providing general nursing care to patients in a practice environment as an autonomous practitioner
- Experience of Health Education and Promotion
- Experience of triaging patients with a range of clinical conditions
- Experience of Long-Term Conditions and Chronic Disease Management
- Experience of supervising and appraising staff
- Experience of working with other organisations
- Experience of delivering training programmes

**Desirable**

- Experience of working in Forensic Mental Health setting

### Qualifications

**Essential**

- First level registered nurse, RGN
- Post Registration diploma or equivalent experience
- Evidence of continuing professional development in relevant areas e.g. Tissue Viability, Continence Promotion, Long Term Condition Management

**Desirable**

- Non-Medical Prescribing Qualification or willing to undertake
- Mentorship qualification
