### Job overview

The in-patient Upper GI Service is based at Gloucestershire Royal Hospital (GRH) alongside day-case and outpatient facilities. Outpatient and day case surgery are also offered at Cheltenham. Outpatient services are also provided at Stroud, Forest of Dean, and the Vale. All specialist Upper GI in-patient and day care is provided on dedicated wards at GRH (Ward 4A / DSU). Emergency surgery is provided on ward 5A.  In addition, benign upper GI and emergency surgery is managed in CGH (Tivoli/ DSU).

### Main duties of the job

The appointment to this post has the following aims:

- To join a team that provides comprehensive consultant-based, patient-centred management of Upper GI patients. The Trust is committed to providing equitable working conditions with colleagues for new appointments.
- To share in a comprehensive rota for emergency and elective general and Upper GI care
- To provide expertise and leadership within the Three Counties Oesophagogastric Centre based in Gloucestershire Hospitals. Including:
- Comprehensive management of benign upper GI surgical conditions
- Specialist Benign Upper GI and general surgery clinics
- 2WW and MDT Cancer clinics
- Diagnostic endoscopy service (with opportunity to develop/acquire specialist endoscopy skills
- MDT attendance.
- Service development
- To contribute to the continuing development of the department’s clinical, research and educational interests. Appropriate additional duties would be incorporated into the post-holders job-plan after initial job-plan review.
- To contribute to the ongoing reconfiguration of GI and emergency general surgery in Gloucestershire.

### Detailed job description and main responsibilities

- Clinical Responsibilities/Key Responsibilities

The appointee will be expected:

- To share responsibility for the delivery of high-quality care to the patients presenting to the department

- To examine and treat patients on the shop floor of the Department
- To support the clinical decision making of junior medical and nursing staff
- To provide on call support out of hours.  To attend the Department out of hours as per agreed protocols. To remain contactable and available when on call
- To carry out ward rounds

- To provide conditions for improved training opportunities in line with national and local recommendations.

- Professional Development, Education and Training
- To participate in the undergraduate teaching programme.
- To assist and participate with the development of postgraduate training for F1s, F2s, SpRs, STs and other staff as appropriate
- Where agreed, to act as Educational Supervisor to junior and middle grade medical staff, being responsible for their appraisal and personal development planning. This would be agreed at initial job-plan review (6-12 months) and reflected in SPA allocation.
- Management and Leadership
- To provide medical information for the development of systems appropriate for Trust needs
- To participate in departmental consultant and senior staff meetings whilst remaining within the framework of the agreed strategy of the Trust and the Department
- To attend other departmental, Divisional and Trust meetings as necessary
- To attend regional and national meetings as necessary
- To undertake all work in accordance with Trust procedures and operating policies

### Knowledge

**Essential**

- Ability to appraise research critically and apply research outcomes to clinical and surgical problems
- Ability to manage risk
- Working knowledge of evidence based practice
- Ability to teach clinical and operative skills

**Desirable**

- Publications in peer-reviewed journals

### Experience

**Essential**

- Ability to offer an expert clinical opinion on problems in Upper GI, including Upper GI Cancer.
- Ability to offer an expert clinical opinion on the breadth of emergency general surgical conditions
- Ability to take full and independent responsibility for clinical care of patients
- Validated logbook indicating adequate and appropriate level of specialist Upper GI surgical operative experience
- Training and experience in laparoscopic Upper GI surgery

### Qualifications

- Full GMC registration
- MBBS or equivalent
- FRCS or equivalent

**Desirable**

- Additional degrees, diplomas or postgraduate thesis relevant to the specialty
