Fixed term for six months pending substantive recruitment.

We are looking for Consultants with a passion for Frailty. The post offers a new challenge with the ability to be involved in designing an integrated Frailty Service, bridging community and secondary care. You must be a capable senior decision maker who will support the rapid assessment of patients including post take ward round for patients, streaming to appropriate pathways and commencement of treatment. There is the possibility of a session of specialty work for those with specialist interests. Applicants wishing to work less than full time or to job share are encouraged to apply.

### Main duties of the job

The Frailty team is a large multi-disciplinary team based in the Frailty Assessment Unit on ward C-16 with in reach into the Emergency Department The team comprises of 1.7 frailty consultants, 1 registrar, 2 lead nurses, 5 clinical nurse specialists and 3 Physicians Associates. The service provides diagnosis, treatment, and facilitates early discharge of frail patients and provides interface with frailty virtual ward (established in November 2022).

We have close links with the Integrated Care Board for Cambridge and Peterborough and are there are plans to work with community partners to develop a Community Falls Hub and other community pathways.

### Detailed job description and main responsibilities

- Work with the Frailty team whilst on direct clinical care.
- Attend Frailty team meetings
- Attend service improvement meetings
- Attend UEC governance meetings.
- Support education programme for junior staff in UEC and frailty team.
- Work closely with the Emergency department team.
- Work closely with the Acute medicine team.
- Take responsibility for own continuing professional development and take part in continuing medical education activities, in accordance with any relevant standards for maintaining GMC licence to practice and membership of any relevant professional body.
- Be committed to the concept of lifelong learning and produce and maintain a Personal Development Plan in agreement with your appraiser.
- Attend relevant local and national activities in order to maintain up to date knowledge and to ensure compliance with legal and procedural requirements associated with the current processes of certification, investigation (by coroners) and registration.
- Participate in any relevant governance activities relating to the scrutiny of deaths.

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

## Person Specification

### Qualifications

**Essential**

- Registered medical qualification. MRCP or equivalent.
- MRCP or equivalent.
- Full registration with GMC

**Desirable**

- Higher degree or diploma (e.g. MD, PhD).
- Name on Specialist Register for Geriatric Medicine
- Within 6 months of the expected date of CCT (or equivalent) on the day of interview.

### Training & Experience

**Essential**

- Wide experience in all aspects of Geriatric Medicine and interest in obtaining further qualifications in post.

**Desirable**

- Experience in front door Geriatrics Medicine and admission avoidance.

### Leadership/Management Skills

**Essential**

- Able to demonstrate leadership capability within multi-disciplinary teams.

**Desirable**

- Management course and/or qualification.

### Knowledge/Skills/Clinical Expertise

**Essential**

- Experience in and knowledge of Geriatric Medicine.
- Thorough understanding of principles of medical audit

**Desirable**

- Wide experience in all aspects of Geriatric Medicine and interest in obtaining further qualifications in post.
- Must have undertaken and completed audit projects
- Experience in front door Geriatrics Medicine and admission avoidance.

### Academic Achievements (including research and publications)

**Desirable**

- Publication of original research papers
- Publication of relevant review articles or case reports.
- Experience of clinical research
- Experience of recruitment to CRN studies
