We are looking for a colleague to rotate through areas covered by geriatricians at the OUH NHS FT. The posts will compromise the following ward areas: general geriatric medicine, acute general medicine, peri-operative orthopaedics at the John Radcliffe and Nuffield Orthopaedic Hospitals and general internal medicine at the Horton Hospital. There will be scope to adapt the ward areas covered by mutual agreement and swaps with colleagues.

The job will include a Geriatric Medicine clinic at the John Radcliffe site.

### Detailed job description and main responsibilities

Perioperative Medicine: OUH has an established perioperative medicine service in the acute Surgical Emergency Unit (SEU) and hip fractures. Over the last few years our services have expanded to provide consultant-led medical perioperative care to the major trauma, elective orthopaedics and the vascular services. The perioperative consultants (who are from a geriatrics, acute medicine, HDU and renal background) are supported by 3 perioperative medicine fellows, a geriatric medicine ST3+, a middle grade medical trainee and the foundation doctors in the surgical specialities.  We also have a fragility fracture nurse specialist that works with our trauma patients.

General Internal Medicine: The Horton Hospital is a District General Hospital in Banbury with an Emergency Department admitting a largely unselected medical take, although some specialist services (e.g. STEMIs, acute general surgery) go directly to John Radcliffe. There is a team of at least 8 general physicians based at the Horton operating a team with ward-based structure, with on-call currently 1 in 8. Unselected acute admissions are around 20/day and inpatients average around 20 (one ward). A consultant is typically supported by a team of doctors comprising an ST3+, IMT/FY2/GPVTS and an FY1.

The AGM medical takes at the John Radcliffe Hospital are consultant team based – you are on call with your team (ST3+, IMT/FY2/GPVTS, FY1 and medical students) for every take that you do – which is extremely popular with the resident doctors as it provides a true sense of firm structure.  The 24-hour period is split into three takes and the consultant retains the patients who are admitted to the short-stay areas. Patients admitted to the Complex Medicine Units or other specialty wards are handed over to those teams. This maintains continuity for over 75% of all patients as the majority have a length of stay of less than 72hours.

Ambulatory Units:  based at the Horton and the John Radcliffe Hospitals, these offer senior clinician-led multidisciplinary assessment and ambulatory treatment. All areas have access to point of care testing as well as plain radiology. You will be expected to coordinate and review the care of up to 60 patients per day and supervise advanced care practitioners and junior doctors.  Direct phone support to local GPs is provided. Over 80% of patients are managed on an ambulatory basis.

Geratology: The Department of Clinical Geratology in Oxford is vibrant and dynamic. We can support all subspecialist interests within Geratology and are developing some new ones with our award-winning interface services.
