The All Age Community Learning Disability Team provides specialist, person centred support to children, young people, and adults with learning disabilities. The team works across the lifespan to offer a seamless, consistent pathway of care. The service brings together a range of professionals — including learning disability nurses, psychologists, occupational therapists, speech therapists, to deliver integrated health and social care within the community. The service focuses on promoting independence, reducing health inequalities, supporting families and carers, and enabling people to participate fully in their communities. You will oversee service performance, workforce management, pathway oversight, and quality governance processes, ensuring compliance with regulatory standards, including those set by the Care Quality Commission.
