For this role, it would be expected that you have a good experience of completing strengths based and evidence-based assessments in line with The Care Act 2014, Mental Capacity Assessments and have knowledge and experience of Safeguarding Vulnerable Adults.  It would also be expected for you to have knowledge of Continued Health Care Assessments and Funding ### Main duties of the job Working in a supportive team, you would be expected to complete assessments within agreed timescales to meet the increased demand of discharges from the acute hospitals and be flexible to your working role.  You will be predominantly working within the community, completing visits within local care homes and be involved with regular multi-disciplinary team meetings to promote and advocate for the individuals and their families and carers.
