### Job overview

We are looking for a dynamic Leg Ulcer and Lower Limb Specialist to join our Lower limb ,lymphoedema and leg ulcer  service at Lewisham and Greenwich NHS Trust. The registered OT, Physiotherapist or nurse specialist will work across the borough alongside the leg ulcer and lymphoedema nurse specialists, District Nursing and wider multi- disciplinary teams to deliver high quality, safe and effective care, ensuring  excellent patient experience.

The post holder will be based at the Waldron Health Centre, New Cross and will be expected to travel within the borough of Lewisham as necessary.

A full UK driving licence and daily access to a car is essential.

### Main duties of the job

The post holder  will provide a safe, effective, high quality specialist service to patients their families and carers and promote self-management of the patient’s condition.

You will take designated responsibilities for a defined caseload and assist teams with assessing patients in their own homes, carrying out holistic assessment, planning, implementation and evaluation of nursing care working as part of the Community Nursing and GPs and Practice Nurses, liaising and working with statutory and voluntary agencies as appropriate.

To develop own specialist clinical knowledge and skills and participate in the development of other staff

To reflect on own practice through clinical supervision/mentorship

To provide a comprehensive nursing service for patients with conditions within the specialty.

To work in close liaison with the multi-disciplinary team across the pathway to meet the needs of patients and their famillies, and to aim for seamless care.

To use clinical judgement and skills in decision making to manage this caseload of patients. To develop skills of history taking and physical assessment to direct appropriate intervention.

To assist in the development of the service

To vigorously promote the patient’s participation in decision-making, enabling informed choice of treatment.

## Person Specification

### Experience

**Essential**

- Minimum two years post-registration experience in relevant field
- Experience of working within the multidiciplinary team
- Experience of liaising with other agencies
- Ability to work autonomously

**Desirable**

- Expertise or willingness to learn and undertake training in the planning and delivery of the care for patients with lymphoedema or other long-term conditions

### Personal Qualities

**Essential**

- Fit for active physical workload and resilient under reasonable pressure of work
- Flexible approach to changing situations

### Knowledge and skills

**Essential**

- Good time management and prioritisation skills
- Excellent interpersonal and communication skills
- Knowledge and understanding of the relevant NICE guidance

### Qualifications and Training

**Essential**

- HCPC, registered Occupational therapist or Physiotherapist Recognised mentoring or teaching qualification ENB 998/FLaP OR Registered Nurse with current NMC registration 1st Level Degree or equivalent
- Evidence of continued professional development of self and other health care professionals

**Desirable**

- Specialist course – Tissue Viability, Leg Ulcer management or Lymphedema
