### Job summary

We are seeking a highly motivated, passionate
Learning Disability Nurse (RNLD) to join the Intensive Support Team (IST) in
Calderdale. We would welcome applications from newly qualified nurses, you
would be supported in your new role under our preceptorship programme.

The IST is a small multidisciplinary
team, working proactively and intensively with individuals with behaviours that
challenge. We provide specialist assessments and interventions to support service
users to remain in their home. We work closely with carers, support networks
and wider agencies to develop their understanding of service users needs. Our
work is informed by the principles of Positive Behaviour Support (PBS). There
will be opportunity to undertake an accredited qualification in PBS.

The IST is part of the Community
Learning Disability Health Team, a friendly and welcoming MDT. There is an
emphasis on multidisciplinary working, with positive working relationships
between professional groups. We have strong and effective links with our
partner agencies including the Local Authority and ICB as well as local care
providers

Based
in Hebden Bridge, a vibrant town on the edge of picturesque
countryside we benefit from free parking in the town centre. We can offer
flexible working hours and an agile working policy, with opportunities for home
working as well as alternative bases to support effective cover of the wide
geographical area of Calderdale. Part time working hours will also be
considered

### Main duties of the job

The post holder
will be responsible for a small caseload of clients of all levels of ability
held jointly across the IST. This involves people presenting with behaviours
that challenge, and responding to service users in crisis, working to prevent
placement breakdown and hospital admission. The post holder will complete
behavioural assessments and work with families, carers, staff teams and other
professionals to implement positive behavioural support plans.

The post holder
will be a member of a supportive multi-disciplinary team where there is an
emphasis on working collaboratively with colleagues. There is also a requirement
to work independently, making autonomous decisions.

A key aspect of
the post is working closely with other agencies to bring about successful
outcomes for our service users.

There will also
be opportunities for continued professional development and also to contribute
to the supervision of junior staff and students where appropriate.
