An exciting opportunity has arisen for an experienced and motivated Learning Disabilities Case Manager to join the Continuing Healthcare (CHC) Team, supporting individuals with learning disabilities across North West London.

This is a rewarding and influential role within a specialist service dedicated to improving the lives of people with learning disabilities and complex health needs. You will play a key role in ensuring individuals receive personalised, high-quality care that promotes independence, choice, dignity and the best possible health outcomes.

Working at the interface of health, social care and commissioning services, you will coordinate complex care packages, undertake assessments, advocate for individuals and their families, and work collaboratively with a wide range of professionals to ensure care is delivered safely, effectively, and in line with NHS Continuing Healthcare guidance.

This role offers an excellent opportunity for an experienced Registered Learning Disabilities Nurse (RNLD) or healthcare professional who is passionate about reducing health inequalities, improving experiences of care, and influencing service development for people with learning disabilities. Development opportunities may also be considered for the right candidate.

### Main duties of the job

As a Learning Disabilities Case Manager, you will:

- Undertake comprehensive NHS Continuing Healthcare assessments for individuals with learning disabilities and complex health needs.
- Develop, implement and coordinate person-centred care plans in partnership with individuals, families, carers and multidisciplinary teams.
- Act as a strong advocate for individuals, ensuring their voice, preferences and aspirations remain central to all decision-making.
- Coordinate, monitor and review complex packages of care, ensuring services remain responsive, effective and focused on achieving positive outcomes.
- Work collaboratively with local authorities, provider organisations, specialist nursing teams and community services to deliver integrated care.
- Support safeguarding processes and contribute to risk assessment and risk management planning.
- Facilitate and lead multidisciplinary meetings, case conferences and care reviews.
- Monitor the quality and effectiveness of commissioned services and identify opportunities for service improvement and innovation.
- Promote best practice in learning disabilities care and contribute to the ongoing development of Continuing Healthcare services.
- Support the organisation's commitment to reducing health inequalities and improving outcomes for people with learning disabilities and their families.

### Experience

**Essential**

- Experience of establishing & maintaining good working relationships
- Experience of working across multi agency services.
- Experience of holistic assessment
- Awareness of Deprivation of Liberty Safeguards -Experience of completing Mental Capacity Act
- An awareness of financial management, especially in relation to brokerage and invoicing
- Experience of completing Mental Capacity Act Assessments

- Registered RN5 Learning disabilities nurse, level 1 with current NMC registration Educated to degree level.
