### Job overview

The learning disability health facilitation team, support people with a diagnosed learning disability to have equitable access to community or acute care.  This exciting and new practitioner role will support and enhance the current acute liaison nurse role in UH Sussex sites to improve the care outcomes for eligible patients. We would consider a job share with two part time applicant's providing practitioner support to both sites.

### Training

- Completed Oliver McGowan Training

**Desirable**

- Level 3 qualification (e.g., A-level, NVQ3) in Health Care or equivalent.
- Care Certificate standards

### Skills and Experience

**Essential**

- Evidence of completion of Oliver McGowan training
- Experience of working with patients in general hospital setting
- Knowledge and experience of the Mental Capacity Act and confidence in encouraging clinicians to consider an individual’s capacity to make an informed decision about their care. Understanding and experience of best interest Performa’s.
- Meeting Administration and taking minutes
- Experience with NHS clinical databases
- Computer Literate and familiar with hospital record management, and Microsoft Office software
- Good written and oral communication skills

**Desirable**

- Evidence in experience in working with, or supporting people with a learning disability
- Knowledge and understanding of the Causes of premature and/or avoidable death for people with learning disability and autistic people (Leder, 2025)
- Experience of working with people in acute general hospital settings and undertaking a health facilitation role.
