### Job overview

The post holder will, with the support of the Pharmacy Senior Management Team, lead all aspects of the medication safety agenda across the Trust and act as the Trust Medicines Safety Officer.

They will work as part of the Pharmacy Board Team to deliver the medicines’ clinical governance plans which support safe and effective Pharmacy and medicines optimisation services to patients under the care of Calderdale and Huddersfield NHS Foundation Trust and other Trusts under service level agreements. This involves close liaison with multidisciplinary teams working across the Trust.

They will develop education and training materials including reports relating to the management of medicines in the Trust and undertake research and audit to support innovative practice and sustain robust medicines management systems and processes.

- Lead the Medication Safety agenda and be responsible for the monitoring systems and provision of information to support a reduction in avoidable harm resulting from inappropriate medicines use across the Trust including a medicines safety dashboard.
- Acting as a role model and expert in medication safety ensuring a consistently high level of pharmacy input in line with corporate objectives and NHS England policy.
- Manage the Pharmacy Medication Safety Team to ensure that the medicines clinical governance agenda is delivered in line with Trust priorities.
- Undertake research and audit to mitigate the risk associated with medicines use.
- As part of the Pharmacy Management team contribute to the strategic development of pharmacy services and lead by example to enhance the Pharmacy Service reputation both at a local and national level.

- Contribute to the local regional and national agenda with respect to medication safety liaising with national regional and local groups to share, learn and minimise medication errors.

### Detailed job description and main responsibilities

- Review and develop systems and processes for monitoring prescribing, procurement, storage dispensing, distribution and administration of medicines which reduce the risk of medication errors.
- Support the development of education and training programmes to enable multidisciplinary teams to discharge their responsibilities relating to the prescription, procurement, supply and administration of medicines.
- Design and develop systems and processes alongside clinical colleagues to mitigate the risk of incidents recurring in the Trust
- Review safety alerts and national safety documents working alongside colleagues to prepare appropriate plans to deliver compliance. Lead serious untoward incident reviews where appropriate and provide a quality assurance function for incident reviews involving medicines. Support multidisciplinary teams to investigate incidents to enable the root cause to be understood and appropriate plans to be established to mitigate future risks. ·       Promote the reporting and monitoring of medication errors within the pharmacy and wider Trust.·       Analyse medication error data from the Trust and from Primary Care and provide information and prepare reports to support risk reduction strategies across the Trust and at the interface between primary and secondary care.·       Identify and review incident trends and work with other healthcare professionals to find and implement solutions to prevent medication errors.·       Lead and support others to implement and learn using Patient Safety Incident Response Framework to ensure the development and implementation of robust action plans in response to medication errors to mitigate the risk of re-occurrence.·       Demonstrate innovation to identify and promote best practice aimed at mitigating the risk associated with medicines use.·       Work with the Trust E-prescribe Team and Pharmacy IT Team to find IT solutions to minimise medication errors.·       To provide highly specialist advice and support to the Trust leads overseeing litigation and claims in the Trust related to medicine safety and where necessary attend MDT meetings /Coroner’s courts. This may include representing the Trust at coroner’s court to explain medication related policies and decisions.·       Lead and develop others to lead the Trust response to national alerts and guidance relating to medication safety and ensure compliance with deadlines.·       Undertake medication related risk assessments and supporting others to prepare assessment where required.·       Support with the management of the Risk Register on behalf of the Pharmacy Department and the Trust for corporate medicines management issues and ensure that action is taken or obstacles to progress are escalated in line with the Trust governance structures.·       Coordinate feedback to staff relating to medication errors and related solutions to all disciplines to minimise future risk and support organisational learning.·       Be responsible for reviewing risk management reports relating to pharmacy or medicines to identify the need for process change or education.

## Person Specification

### QUALIFICATIONS / TRAINING

**Essential**

- MPharm or equivalent Degree in Pharmacy approved by General Pharmaceutical Council
- Registered Pharmacist with the General Pharmaceutical Council
- Postgraduate Diploma in Clinical Pharmacy (or equivalent qualification)
- Pharmacist Prescriber

**Desirable**

- Specialist training and competencies in medication safety or governance or relevant short courses taught at postgraduate level

### KNOWLEDGE, EXPERIENCE & EXPERTISE

**Essential**

- 1 year pre-registration training
- Highly specialised knowledge of medicines related governance with attention to detail
- Extensive experience as a practicing Senior Clinical Pharmacist within a Hospital Pharmacy with demonstrable experience in managing medication safety.
- Evidence of high-level investigational skills with the identification of key learning
- To demonstrate the ability to lead and influence others and implement change
- To have a working knowledge of strategies to implement national guidance for safe medicines use and prevention of medication related Never Events
- Computer literate
