### Job overview

We are seeking an experienced nurse leader to shape and improve palliative and end of life care across community and integrated services. This strategic role will lead service development, improve quality and patient experience, and drive integrated, multidisciplinary care closer to home.

The successful candidate will be a Registered Nurse with expertise in palliative care, strong leadership experience, and a passion for compassionate, person‑centred care, aligned with UHCW values.

The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.

If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy

A full UK driving license is essential for this post. Please note that an international driving license is not accepted.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

- Provide senior professional leadership and corporate accountability for palliative and end of life care services across Integrated Care and Community Services.
- Lead the strategic clinical governance framework for palliative and end of life care, ensuring alignment with PSIRF, quality surveillance, risk management and assurance reporting.
- Act as the expert advisor within community group and system delivery on palliative and end of life care, influencing strategic decision‑making, pathway standards and policy development.
- Analyse and interpret complex clinical intelligence, performance data and trends to inform strategic planning, service assurance and risk mitigation.
- Identify, manage and escalate risks, ensuring shared accountability and multidisciplinary oversight.
