This strategic role will lead service development, improve quality and patient experience, and drive integrated, multidisciplinary care closer to home. The successful candidate will be a Registered Nurse with expertise in palliative care, strong leadership experience, and a passion for compassionate, person‑centred care, aligned with UHCW values. The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group. However, any formal changes will be preceded by appropriate consultation in line with organisational policy A full UK driving license is essential for this post.
