At the Dudley Group, we strive to provide excellent care for our caseload of approximately 160 children and young people with diabetes. Our shared team purpose is: - to provide a high quality, evidence-based, patient centred service. - to work collaboratively as a multi-disciplinary team - to develop confident and empowered individuals who are physically and emotionally healthy. We understand that a diagnosis of diabetes is life-changing. We are here to support our patients and their family on the journey from diagnosis through to transition. The successful candidate will share this vision of empowering and motivating children/young people and their families to achieve the best health outcomes. You will also be keen to be part of service development, embracing the team’s culture of quality improvement. You will have excellent communication skills allowing you to deliver an expert service and ensure that children/young people and their families remain at the centre of what we do. You should be passionate about the role and strive for excellence. ### Main duties of the job To provide specialist advice, support and care to the children, young people and their families with diabetes at the Dudley Group Hospital NHS Foundation Trust, within hospital, home and community settings. To enable children and young people with diabetes to reach their optimum level of health, independence, social and emotional well-being, through the provision of ongoing clinical management, education and support tailored to individual need. To develop and maintain programmes of education for children/young people with diabetes and their families. To act as the clinical expert and professional resource to children/young people and families, and the multi-professional team around them - both within and external to the Trust.
