*INTERNAL MPFT APPLICANTS ONLY*

The post holder will work as part of a Community Nursing Team to provide nursing care without direct supervision for patients registered with GPs within Midlands Partnership NHS Foundation Trust, covering Stafford North PCN. The post holder will participate in the ongoing assessment, planning, implementation and evaluation of programmes of care adhering to policies and procedures.  Strict confidentiality regarding all information concerning patients, staff and sensitive Trust business will be maintained at all times, in accordance with Caldicott Guidance and Data Protection Act 1998.  To participate in the provision of effective pre and post registration learning experience for students and new members of staff. The Post is advertised for those with the Specialist Practice in District Nursing but will consider those currently studying for and near completing this qualification.

### Main duties of the job

The postholder will hold responsibility for a defined caseload of patients drawn from a GP practice population.  He/she will hold a specialist practice qualification and be skilled in assessment of patient care needs and in the development, implementation and evaluation of care packages and acquire and employ active nurse prescribing skills.  He/she will work without supervision, and will, additionally, assist the Operational Lead by conducting delegated tasks instrumental in team management and care co-ordination.

The postholder will assume responsibility for directing team co-ordination.

The postholder will lead in organising the induction and clinical supervision of students and new team members.

The postholder will accept the responsibility for doing a quality job, aspire to achieve, share a corporate pride in achievements and strive to act as an ambassador for good quality.
