Based in the stunning county of Suffolk, you will hold a key role to support the development of integrated working across Health and Social Care through the development and management of the Integrated Neighbourhood Teams, including the associated Connect.  You will have joint accountability to Adult Social Care in Ipswich and East Suffolk and to East Suffolk and North East Essex Foundation Trust (ESNEFT). Collocated in Constantine House Ipswich, this role covers the geography of West Ipswich. There are 8 Integrated Neighbourhood Teams across Ipswich and East Suffolk bringing Health and Social Care Services together to provide a single coordinated care response that is underpinned by prevention, self-care, early intervention, reablement and rehabilitation rather than longer-term treatment and lifelong service dependency. The role will be key in building relationships within the Connect community.
