We are looking for a highly motivated and enthusiastic pharmacy technician to work within our Community Independence Service (CIS) in Central and North West London NHS Foundation Trust.

CIS is a provider partnership which focuses on hospital admission avoidance where clinically appropriate care can be provided in the community. CIS facilitates early supported discharge from hospital, maximises independence and reduces dependency on longer term services. CIS operates across the three boroughs of Westminster, Kensington & Chelsea and Hammersmith & Fulham.

The pharmacy technician will provide an efficient medicines optimisation service and work collaboratively with the Integrated Care Pharmacist to support Urgent Community Response, Home First and Rehabilitation teams to optimise medicines related aspects of care for vulnerable adults with acute or complex medication needs to avoid hospital admissions.

### Main duties of the job

The pharmacy technician will:

- Conduct domiciliary visits to patients in Westminster and Kensington & Chelsea boroughs to provide a medicines optimisation service

- Conduct medicine reconciliation
- Assess medicines related needs, support adherence and non-compliance, and facilitate implementation of safe handling and administration of medicines
- Demonstrate collaborative working with the wider health and social care multidisciplinary team to reduce medication related risk to patients in their own homes
- Participate in the delivery of medicines optimisation initiatives
- Participate in audits within the Trust

## Person Specification

### PREVIOUS EXPERIENCE

**Essential**

- Post qualification experience in hospital pharmacy
- Experience of working on wards, counselling patients, checking PODs, and medicines reconciliation
- Portfolio demonstrating competency of previous and current pharmacy practice skills, knowledge, and CPD
- Education and training of staff
- Experience of supporting or developing pharmacy technician services

**Desirable**

- Experience in a supervisory capacity
- Experience working within care of elderly services
- Working in an integrated care (or equivalent) setting
- Post qualification experience of a medicines optimisation role to care of the elderly
- Primary care pharmacy work experience

### EDUCATION AND QUALIFICATIONS

**Essential**

- Registration as a pharmacy technician with the GPhC
- BTEC/NVQ Level 3 in Pharmaceutical Sciences or equivalent
- HEE Medicines Optimisation Programme (MOP) or equivalent qualification
- Practical experience and theoretical study / knowledge (as evidenced by CPD) in Medicines Management

**Desirable**

- Medication Counselling Training
- Membership of APTUK
- Final Accuracy Checking certificate (ACPT)
- Project Management qualification
- Practice Supervisor qualification

### SKILLS, KNOWLEDGE, ABILITIES

- Ability to work alone and as part of a team

- Good presentation Skills

### ATTITUDES, APTITUDES PERSONAL CHARACTERISTICS
