An exciting opportunity has arisen for a highly motivated and enthusiastic social worker to work within the new Complex Psychosis Pathway (CPP) in Leeds. The Complex Psychosis Pathway provides a whole system approach to Mental Health Rehabilitation and Assertive Community Treatment in Leeds which includes Newsam Ward 5, Asket Croft and Asket House inpatient settings, the Community Rehabilitation Team and Assertive Outreach Team.

We provide care to people living with a psychotic illness with severe and treatment resistant symptoms and other related serious comorbidities that severely affect the persons social and everyday functioning. Our pathway provides flexibility and support over the longer term to improve their mental health, psychological wellbeing, social inclusion and maximise their quality of life.

The post holder will work in a way that promotes positive partnership working between all members of the Rehabilitation and Recovery service Complex Psychosis Pathway and its partners.  They will have an excellent understanding of the principles of recovery and will practice clinically in a way that promotes therapeutic optimism and hope with service users.

### Main duties of the job

The post holder will oversee the social care needs of service user group across the inpatient areas of the Complex Psychosis Pathway. To provide an innovative professional social work service to in-patient service users within the Complex Psychosis Pathway Rehabilitation Services while promoting social inclusion.

Please note in Winter 2026, Newsam Ward 5 will be relocating to our own redeveloped site located at Parkside Lodge, Stanningley Road, Armley, Leeds.

### Detailed job description and main responsibilities

The post holder will be expected to participate in multi-disciplinary and multi-agency working, being aware of colleagues' caseloads and providing support to others as necessary and contribute to the on-going development of complex rehabilitation.

Work in partnership with Service Users, Ward Staff, and partner agencies (e.g. housing, Benefits, Social Services etc.) to facilitate benefit claims and other financial and housing support.

Ensure the service user and their families are central to and fully involved in the plan of care.

Ensure effective communication and collaboration with others through attendance at review meetings, team meetings, ward rounds, clinical meetings and meetings held by partner agencies.

Support Carers Champions to support them to support Carers.

Support with liaising with Local Authority to complete Care Act Assessments.

Support Carers to access Carers Assessments, supporting carers to make contact with Local Authorities.

Complete Statutory Social Circumstances reports for mental health tribunals and/or hospital manager’s hearings. To complete other reports as required.

Maintain comprehensive and up to date written records of assessments, care plans, evaluations, and on-going process notes in line with Trust Policy and to share these with the MDT and external agencies as appropriate.

Link in with the Social Work leads within the CPP and Social Work pathway within the Trust.

Support with Social Work student placements.
