### Job summary

Are you an Occupational therapist looking for an opportunity to work with patients on their rehabilitation goals. Alton Community Hospital are looking for a Band 5 (will consider apprenticeship route) to join their passionate therapy team. The therapy team focuses on patient centred care for individuals who have suffered an acute episode and require rehabilitation to improve their quality of life. The teams are responsive, well led, and caring providing guidance to patients, relatives, carers and staff.

We are calling for band 5 to apply to work within our innovative Therapy Team, striving to embed the rehab access for all campaign. The service is run by a multi-disciplinary team and you will work with other professionals to provide the patient with the best possible outcome. You will have the opportunity to develop your clinical, communication and leadership skills whilst working with patients, carers, relative and staff. You will be encouraged to show your creativity and innovative side whilst supporting the team lead in the development of the service.

### Main duties of the job

To provide clinical assessments of patients who have been admitted to the ward, with a variety of conditions from respiratory, neurological, orthopaedic and MSK origins. You will use the assessment to formulate a goal orientated treatment plans to be carried out as an autonomous practitioner.

To be responsible for own case load and co-ordinate interventions with other disciplines and external agencies as required.
