Join Our Integrated Discharge Team – Make a Real Difference Every Day

We’re looking for a bright, motivated and hardworking nurse to join our energetic and supportive Integrated Discharge Team at Broomfield Hospital on a 12‑month secondment / fixed‑term basis.

In this rewarding role, you’ll be central to helping patients move safely and smoothly from hospital to home or onward care. You’ll assess patients who are medically optimised for discharge, identify their ongoing care needs, and coordinate timely referrals to community and system partners—ensuring every discharge is safe, effective and patient‑centred.

Strong communication is at the heart of what we do. You’ll work closely with patients, families, multidisciplinary teams and wider system partners, using your clinical judgement and interpersonal skills to remove barriers to discharge and maintain effective patient flow across the hospital.

### Main duties of the job

Work with patients and their families who have complex needs and conditions to support timely, safe and effective discharge into the community.

Provide professional guidance and support to patients and carers throughout the discharge planning process.

Develop and coordinate holistic discharge plans in partnership with patients, families and multi‑agency colleagues.

Ensure discharge interventions are appropriate, coordinated and not duplicated across services.

Support patients with complex healthcare needs, including those requiring long‑term health and social care planning or short‑term intervention following acute illness or deterioration.

Anticipate, coordinate and integrate health and social care needs for patients at high risk of unplanned hospital admissions.

Work at the centre of integrated care planning within Broomfield Hospital, alongside Social Care, voluntary organisations and other external partners.

Act as a key point of contact for liaison and communication between the hospital and community services, including informal and formal carers.

Facilitate smooth, safe and effective transfers of care from hospital to home and between community and acute settings.

Contribute to improved quality of care and patient satisfaction through effective discharge coordination.
