### Job overview

An exciting opportunity has arisen within the Transplant directorate for a pathway tracking coordinator to join the team.  You will navigate patients through their care pathway, from point of referral to treatment in accordance with the national targets. Close attention to detail in updating patients, notes, records and tracking RTT pathways.

### Main duties of the job

Responsible for validating, amending and improving data quality in relation to medicine pathways from initial referrals, through diagnostics to completion of patients’ episode. Liaising and co coordinating with all involved in the patient’s management.

Accessing and updating EPIC, ensuring that national and local targets are met and reported on at the divisional level and relevant PTLs.

Due to Home Office immigration rules, a full time permanent vacancy cannot be filled by individuals on a Student visa. Therefore, please be advised that if you are a Student visa holder, we will not be able to offer you a full time permanent contract unless you have:

-       applied for a Graduate visa

-       or you will have successfully completed your course and have applied for a Graduate visa before the anticipated start date of your employment

This vacancy will close at midnight on 28th June 2026.
