This is a unique opportunity to deliver high quality, person centred podiatric care to individuals who face significant barriers to accessing healthcare. The post holder will act as a key link practitioner between the Homeless Health Service and Podiatry Service, supporting continuity of care and improving access to appropriate services. The role involves autonomous assessment, diagnosis and management of patients with a wide range of complex podiatric and medical conditions, including high risk and at risk foot care. The post holder will also provide specialist advice, education and support to colleagues and the wider multidisciplinary team. Working in partnership with internal teams and external organisations, including community services, GPs, hostels and outreach centres, you will contribute to reducing health inequalities and promoting inclusion within healthcare services.
