### JOB SUMMARY

Homecare Services

The Directorate of Primary Care, Older People’s Services and Nursing provides a range of homecare services designed to enable service users to remain at home and to live as independently as possible. Practical care services provision is achieved through empowering service users and carers to build on existing support networks. The focus of the Homecare Service is the provision of high quality care services in line with regulation and minimum required standards.

Team Coordinators are an integral part of the team to which they are attached, they are expected to work closely with the Area Managers and other staff to ensure the delivery of a high quality service. Team Coordinators are office based and required to undertake the mainly electronic administrative tasks which underpin the delivery of good quality homecare so attention to detail is paramount. Frequently Team Coordinators are the first point of telephone contact for members of the public, Homecare workers and community based colleagues and must understand the implications of representing South East Trust in this way. The post holder will be expected to use/her own initiative in carrying out a range of duties involved in the placement of Homecare workers with service users.

### KEY DUTIES / RESPONSIBILITIES

1 Process referrals for Care Services and arrange replacement service for service users when Homecare workers are unavailable due sickness, training, annual leave or any other type of leave.

2 Make and receive telephone calls and emails and take appropriate action. This involves principally contact with Homecare workers, service users and other professionals with regard to service delivery arrangements.

3 Accurate and clear notification to Homecare workers by telephone, electronically or in writing of the service they are to provide to service users. Responsible for preparing, checking and circulation of care rotas.

4 Undertake work allocated from Area Managers and other senior staff.

5 Provide relief cover for other Team Coordinators and provide cover for the out of hours service provision (office based).

6 Inputting all relevant data concerning service delivery, promptly and accurately, to Electronic information systems - CareLine Live, Encompass, E-Rostering etc, in accordance with Trust and Homecare Policies and Procedures.

7 Liaise with other disciplines, departments and external agencies in a helpful and professional manner.

8 Comply with all current Trust and Homecare Policies and Procedures.

9 Provide Payroll with the hours worked by Homecare workers.

10 Participate in the Induction and Training and Development of Homecare workers and maintain Homecare workers personal files and records.

11 Miscellaneous duties including opening and distributing mail, filing, photocopying, stock ordering to ensuring effective administrative support.

12 Preparation of Statistical Information.

13 Dealing with requests for information.

14 Organising and on occasion participating in team meetings.

15 Participate in the Trust’s Performance and Development Process and supervision in line with policy.

16 Attend mandatory training and undertake any further training identified.

17 Undertake special projects as required.

18 To monitor, update and revise systems and procedures as necessary.

19 To monitor the hours worked and mileage claimed by Homecare workers to ensure they are paid correctly and to report issues to Domiciliary Area Manager.

20 Liaise with the Area Manager to ensure that the contracted hours for all staff are accurately maintained.

21 To monitor and order stock for office and Homecare workers and ensure that all equipment databases are maintained.

22 Carry out duties of a similar nature and appropriate to the band, as directed.

23 To assist team members in evidencing compliance with all Corporate Quality Assurance Schemes eg ISO, IIP.

24 To be aware of and ensure both self and staff from whom responsible working in accordance with Trust Policy and procedure and regulation.

25 To be aware of and compliant with the Domiciliary Care Agencies Regulations (Northern Ireland) 2007 and RQIA Domiciliary Care Minimum Standards.

26 To be aware of the organisations aims and objectives outlined in the corporate plan.

27 To ensure compliance with own mandatory training and any training needs identified.

28 To assist with the provision of appropriate and timely data for directorate Governance requirements.
