The Hospital at Home team also provides an Urgent Community Response (UCR) function, offering rapid assessment, diagnostics, and treatment for people with complex health needs, frailty, or exacerbations of long-term conditions. Patients referred to the service are triaged to be seen within 2 hours, or same day for urgent assessment. ### Main duties of the job Clinical Assessment & Diagnosis Act as an autonomous clinician, performing comprehensive history-taking, physical examinations, and clinical assessments in patients' own homes or care homes.Diagnose and manage undifferentiated, acute, and complex conditions within scope of practice.Undertake and interpret diagnostic tests (e.g., ECG, urinalysis, blood tests, point-of-care testing) to inform clinical decision-making. Provide both urgent and routine care, including prescribing (if qualified) or recommending appropriate treatment plans in collaboration with the GP or clinical team.Develop and implement management and treatment plans for patients with acute illness, exacerbations of long-term conditions, or frailty-related deterioration.
