### Job summary

This is a dual role working across two complementary services, a GP home visiting service and the Hospital at Home (H@H) Frailty Virtual Ward. The post holder will provide timely, high-quality care to patients in their own homes or care homes, supporting both short-term acute needs and longer-term interventions that help patients remain safely out of hospital.

Working independently and autonomously, you will deliver face-to-face assessments, make safe and effective clinical decisions, and provide expert care within your professional boundaries and scope of practice.

Within the Hospital at Home Frailty Virtual Ward, you will support patients who would otherwise require hospital admission, delivering acute treatment and monitoring in their usual place of residence. This model of care helps reduce deterioration and avoids unnecessary hospital stays.

The Hospital at Home team also provides an Urgent Community Response (UCR) function, offering rapid assessment, diagnostics, and treatment for people with complex health needs, frailty, or exacerbations of long-term conditions. Patients referred to the service are triaged to be seen within 2 hours, or same day for urgent assessment.

### Main duties of the job

Clinical Assessment & Diagnosis

Act as an autonomous clinician, performing comprehensive history-taking, physical examinations, and clinical assessments in patients' own homes or care homes.Diagnose and manage undifferentiated, acute, and complex conditions within scope of practice.Undertake and interpret diagnostic tests (e.g., ECG, urinalysis, blood tests, point-of-care testing) to inform clinical decision-making.

Provide both urgent and routine care, including prescribing (if qualified) or recommending appropriate treatment plans in collaboration with the GP or clinical team.Develop and implement management and treatment plans for patients with acute illness, exacerbations of long-term conditions, or frailty-related deterioration.

Respond to same day visit requests for acute illness, minor injuries, or worsening of chronic conditions, ensuring safe and effective care delivery in the patient's usual place of residence.Conduct home visits for patients unable to attend the practice, including those with frailty, complex needs, or acute deterioration.Deliver care as part of the Hospital at Home (H@H) Frailty Virtual Ward, providing treatment to include IV therapy, monitoring, and review at home for patients who would otherwise require admission.
