To support delivery of our clinical pathways we are investing in a range of psychological practitioner roles, which will sit within/work across our new community neighbourhood teams. As a Trust we pride our-selves on our innovative and collaborative approach to providing the highest quality, person-centred care: As part of our transition to an integrated neighbourhood model of care, we are developing clinical pathways across primary, secondary care and crisis and emergency service. Our pathways embody strong partnership working with a wide range of statutory and VCSE providers and people with lived experience.
