The role sits at the heart of a fast-paced, multidisciplinary environment, providing expert clinical leadership to support some of our most complex and vulnerable patients. You will play a pivotal role in driving high-quality, person-centred care, influencing pathways, and enhancing outcomes for individuals with frailty across the acute setting. This role offers the opportunity to develop and embed best practice, strengthen multidisciplinary working, and contribute to strategic service improvements within a pressured but rewarding environment. You will be required to work effectively in collaboration with the multidisciplinary teams, including other health professionals to promote the development, planning and delivery of intervention programmes and training in the specialist clinical area. Participation in service projects and audits is encouraged. You will be expected to cover weekend working rota.
