We are seeking a Highly Specialist Occupational Therapist (OT) to join our dynamic Multi-Disciplinary Team (MDT) in providing a seamless, patient-centred service.

In this role, you will develop, implement, and evaluate the Merton Enhanced Home-Based Rehabilitation Team (MEHBRT), ensuring that patients receive timely, high-quality care that supports their independence and wellbeing.

You will assess referrals from hospitals for patients ready for discharge who continue to have health or social care needs. As a clinical leader, you will provide guidance and support to occupational therapists, junior staff, rehabilitation support workers and MDT students, fostering a culture of learning, collaboration and excellence.

This role involves managing your own complex caseload, contributing to service quality and performance, and building strong, inclusive partnerships with statutory, voluntary and independent organisations. You will deliver compassionate, competent care to patients with physical and cognitive difficulties in both community and care home settings.

## Person Specification

### Experience

**Essential**

- Appropriate post-qualifying experience in a range of areas acute care of the elderly, neurology and community
- Appreciation of constraints to working in the community
- A clear understanding of current NHS issues, professional standards, CPD, clinical governance and experience of audit
- Previous NHS experience
- Ability to assess patient conditions, establish the patients problem, formulate and implement a treatment plan
- Has sound knowledge of the Mental Health Act and other legislation
- Experience of MDT working and understand their roles
- Experience of supervising juniors/assistant/students
- Experience in research / literature reviews
- Experience in community work
- Post registration experience at band 6 level in a wide range of clinical areas including elderly rehabilitation, community rehabilitation, falls management, dementia care, respiratory care and orthopaedics
- Experience in quality improvement and service evaluation.

### Education/Qualification

**Essential**

- Degree or diploma in Occupational Therapy
- Registered with the Health Care Professions Council
- Up to date relevant knowledge through CPD as evidenced by portfolio

**Desirable**

- Membership of Royal College of Occupational Therapy and / or relevant special interest groups
- Attendance at Clinical Educator Course
- Leadership and management training
