### Job overview

The Hepatology Pathway Navigator will play an integral role in promoting rapid cancer diagnosis through a robust pathway of surveillance. Working under the guidance of registered health care professionals, this role is to ensure that our patients with liver disease who are at great risk of developing liver cancer are kept under hepatocellular surveillance and that surveillance investigations are carried out in a timely manner.

### Main duties of the job

The pathway Navigator will be expected to coordinate such investigations and track the progress of patient care through the diagnostic phase, pursuing results and ensuring all results are managed appropriately to avoid potential delay.

Maintain and update relevant databases as required

Liaising with other departments within the trust to ensure investigations, opinions, and treatments are sought in a timely manner.

To help reduce patient cancellations, non-attendance for tests and clinic appointments therefore influencing progress through rapid diagnostic pathway

To undertake a range of administrative duties which facilitate and support coordination of care pathway steps, including assisting in the arrangement of appointments, tests.

To work collaboratively with the wider MDT in the navigation of pathway steps.

To escalate issues of pathway delay in a timely fashion in accordance with pathway milestones and organisational procedures and guidelines.

To maintain accurate documentation related to patient care and interactions both written and electronic in line and to comply with organisational policies relating to information governance.

To actively engage in service improvement actions across the pathway which maximise patient experience and outcomes.

- To ensure that our patients with liver disease who are at risk of developing liver cancer are kept under Hepatocellular surveillance.

- To ensure that surveillance investigations are carried out in a timely manner.

- To track the progress of patient care through the diagnostic phase

- To act as an advocate and role model for person centred care across all components of the pathway to positively influence the approach and behaviours of other care/support providers.

- Frequent concentration pattern interruptions

- Checking data quality

- Communications and Working Relationships

- It is essential to this role that the postholder establishes and maintains a good working relationship with a wide range of NHS staff.

- Communicate verbally and in writing to members of health and social care teams

- Participate in meetings, email, telephone conversations ensuring a two-way flow of information

- Disseminate information as appropriate

- Liaise with other members of the MDT team

- Promote and share ideas

- General Working Conditions

- Requirement to use PC for long period of time

- Development of effective working relationships to ensure cooperation and confidence of clinical and non-clinical staff.

### Experience

- Experience working in an office environment.
- Experience of inputting accurate and timely data into computer systems/databases
- Experience of working with confidential information
