### Job overview

Heart Failure Specialist Nurse

An exciting opportunity has arisen for a motivated Heart Failure Specialist Nurse to join our dynamic nurse-led Heart Failure Service based at Royal Oldham Hospital.

In this autonomous role, you will deliver specialist assessment and management of patients with heart failure, using advanced clinical skills to provide high-quality, evidence-based care. You will independently order and interpret cardiology investigations, develop personalised management plans, and work closely with cardiologists and the wider multidisciplinary team.

You will play a key part in service development, audit, research, and training, sharing expert knowledge to support colleagues, patients, and carers. Flexibility to work across sites and a commitment to continuous improvement are essential.

About you:

- Non-Medical Prescriber
- Level 7 Clinical Assessment qualification
- Heart Failure module
- Strong knowledge of heart failure management and evidence-based practice

This is an excellent opportunity to work in a highly specialist, forward-thinking service, where you can influence patient care and contribute to ongoing innovation and improvement.

### Main duties of the job

The successful applicant will manage a defined caseload of heart failure patients, working autonomously to assess, prescribe, and optimise treatment in line with local and national guidelines. You will provide integrated specialist support across hospital and community settings, offering expert advice to patients, carers, and healthcare professionals to ensure high-quality, patient-centred care throughout the pathway.

A key aspect of the role is leading on education and workforce development. You will identify learning needs across the multidisciplinary team and design, deliver, and evaluate training programmes covering the full spectrum of heart failure management—from diagnosis to supportive and palliative care. This will include facilitating study days, delivering clinical teaching, and supporting both classroom and practice-based learning.

You will act as a mentor and assessor for learners, helping to develop knowledge, skills, and confidence within the team. By fostering a positive and effective learning environment, you will address gaps in practice and support continuous improvement in patient care.

In addition, you will contribute to wider education strategies, using your communication and teaching skills to enhance understanding of the complex needs of heart failure patients and promote best practice across services.
