### Job summary

We are seeking a full time enthusiastic cardiac Nurse to join our busy community Heart Failure team to help us provide exceptional Patient-centred care. You will work within our specialist service playing a crucial role in Patient assessments and supporting the delivery of individualised management plans at home or in clinic settings.

As part of a skill mixed team you will be committed to keeping Patients out of hospital and in their preferred place of care. Youll work closely with a skill mixed specialist team to provide holistic Nursing care, education, and support to Patients and their families. Your expertise will help improve Patient outcomes, enhance quality of life and the Patient experience.

### Main duties of the job

To work in partnership with the Heart Failure Specialist Nurse team in providing specialised evidence based care in accordance with the clinical management plan by working in conjunction with the senior HFSNs. The aim is to provide care and interventions until the Patient reaches stability. To work independently managing a caseload of Amber patients and prioritising clinical need by using specialist assessment skills and evaluations. To assist in the promotion of early identification and interventions to reduce episodes of decompensation. By working closely with the HFSN and responding proactively to changes in condition and symptoms of fluid retention.

- Relevant learning beyond registration
- Specialist up to date expertise acquired from theory acquired through CPD
- Knowledge of local policies, practices and procedures within specialist area and a good understanding of national strategic context of the NHS and knowledge of NICE guidelines.
- Evidence of proactive involvement in the teaching and mentorship of learners.

- Understanding of community working

- Substantial post registration cardiac experience and understanding of cardiac conditions

- First degree in nursing or an equivalent portfolio of evidence

- Post basic qualification in cardiac related subject/ heart failure

- Cannulation/venepuncture
- ECG and interpretation
