An excellent opportunity has arisen for a dynamic and enthusiastic Band 7 Heart Failure Nurse Specialist with a broad range of clinical experience to join our team. The role requires a flexible approach to work with commitment and the ability to prioritise workload in the fast-paced environment.

The role of the Band 7 Heart Failure Clinical Nurse Specialist is to provide specialist advice and guidance to clinical teams, patients and carers in the inpatient and community settings. Their role includes; direct clinical assessment, medication prescription and detailed specialist guidance for the MDT caring for a patient with confirmed heart failure. Essential to the role is excellent written and communication skills and the ability to work both autonomously and within the broader heart failure MDT.

The ideal candidates will be Independent Prescribers who are experienced in Cardiology. However, candidates who do not meet the full personal specification would be considered for a Band 6 developmental position (moving to Band 7 on completion of specialist competencies) if they have other relevant experience that may be advantageous to this role.

You will have access to study and training opportunities to further enhance your skills and knowledge and will be able to share your expertise through teaching colleagues and providing patient/carer education.

### Main duties of the job

The aim of the post is to facilitate the early identification of patients admitted to hospital with heart failure by the detection and gathering of patients already known to have heart failure or patients who are admitted with heart failure.

To provide clinical leadership, advice, support and information to staff in respect of all aspects of heart failure care to all staff groups. The role will include the development of care pathways, improved discharge planning and improvements of discharge information. The post holder will contribute and maintain links across primary and secondary care.

The post holder will be a key member of the Heart Failure Team, providing specialist-nursing care to all patients referred to the Heart Failure service, utilising advanced nursing practice skills and knowledge to enable streamlining of the care pathway.

The post holder will act as a source of expertise and knowledge for patients, relatives and health professionals.

They will be expected to liaise with the MDT to provide assessment and follow-up of patients with a confirmed diagnosis of heart failure - ensuring optimal therapy and promotion of education towards self-management. They will be expected to liaise with GPs, community services and social services to develop a multi-disciplinary approach towards improving the care and management of patients with a diagnosis of Heart Failure.

### Experience

**Essential**

- Extensive post registration experience. (Band 6&7)
- Relevant experience within a cardiology setting (Band 6)
- Ability to work, motivate and develop a professional team (Band 6&7)
- Relevant experience at Band 5 within a cardiology/cardiothoracic setting (Band 7)
- ECG Interpretation or working towards (Band 7)

**Desirable**

- Specialist training relevant to role: ALS (Band 6)
- Specialist training relevant to role: Counselling skills (Band 6&7)
- Experience as a band 6 heart failure specialist nurse (Band 7)

### Education and Qualifications

**Essential**

- RGN Level one (Band 6&Band 7)
- NMC registered (Band 6&Band 7)
- To have completed the national PEAP programme for practice supervisors or equivalent teaching and assessing qualification for those with ENB998. (Band 6&Band 7)
- Continuous professional development (Band 6&Band 7)
- First degree or working towards(Band 6&Band 7)

**Desirable**
