Do you want to make a real difference in your local community? \</em\>If so, wed love to hear from you!\</p\>\<p\>Join HCRG Care Group as a \<strong\>Healthy Family Practitioner\</strong\> based in Preston and play a vital role in supporting children, young people, and families across Lancashire.\</p\>\<p\>Working as part of the \<strong\>Young People & Healthy Families Service\</strong\>, youll collaborate closely with Health Visitors, School Nurses, and wider multidisciplinary teams to deliver high-quality, compassionate care. This is a rewarding community-based role where youll empower families, promote child development, and help improve long-term health outcomes.\</p\>\<p\>This is a part-time fixed-term opportunity working 30 hours per week with a planned start date of 01/09/26 31/08/27.\</p\>\<p\>\<strong\>What you\\'ll be doing:\</strong\>\</p\>\<p\>In this varied and impactful position, you will:\</p\>\<ul\>\<li\>Deliver key elements of the \<strong\>Healthy Child Programme\</strong\>, including developmental reviews (e.g. 1-year and 22.5-year checks)\</li\>\<li\>Support children and families with \<strong\>targeted interventions\</strong\> such as behaviour management and infant feeding\</l
