### Job summary

21 Hours per week

We are looking for an enthusiastic and motivated Therapy Support Worker to join The Rotherham NHS Foundation Trust Locality Therapy Service. This role sits within the Routine element of the service and involves working in patients homes, responding to referrals from health professionals for individuals requiring rehabilitation.

You will be part of a well-established multidisciplinary team, supporting Patients to maintain independence and maximise their function within their own home environment. This is an excellent opportunity for someone with a passion for rehabilitation and community-based care.

### Main duties of the job

You will support Physiotherapists, Occupational Therapists and Assistant Practitioners in delivering rehabilitation, discharge planning, and crisis management within patients homes. This will include issuing and fitting equipment, supporting mobility and exercise programmes, and assisting Patients with activities of daily living, including personal care where required.

You will work both independently and collaboratively within a multidisciplinary team, communicating effectively with Therapists, Nurses, social workers, Carers, and other professionals. The role requires the ability to prioritise workload, respond to urgent situations, and maintain accurate clinical records using IT systems.

You will demonstrate sound judgement, recognising when to seek guidance, and contribute to safe, effective care through risk assessment and adherence to clinical governance and health and safety principles.

The role requires experience of working in a rehabilitation, reablement, or community setting and the ability to support patients with complex physical, emotional, and psychological needs. You will also be expected to complete all relevant training and achieve competency within the first six months.

The Trust delivers a 7-day service, and although the role is primarily Monday to Friday, flexibility is required to meet service demands.

**Essential**

- Demonstrates skills & ability in use of computer packages on a regular basis

### Knowledge

**Essential**

- Demonstrates their understanding of the role of the support worker in rehabilitation / management of patients within the community setting
- Demonstrates knowledge of equipment available and able to assess and fit where applicable
- Able to recognise when guidance and support is required

**Desirable**

- Understanding of the importance of Health and Safety in the workplace
