Join Hampshire and Isle of Wight Healthcare NHS Foundation Trust as a Healthcare Support Worker!

Our aim is to deliver high quality, safe services which improve the health and wellbeing of the people we care for. Your contributions make an immediate difference to the lives of the people we serve, whilst helping to avoid unnecessary hospital admissions.

We're looking for individuals to join our Hospital at Home and frailty service, with vacancies across Eastleigh, Lymington and Waterside. This is a fast paced role, where you'll be delivering outstanding care in patients' homes. No two days are the same; from performing basic health observations and supporting rehabilitation, to helping patients with personal care, mobility support and emotional wellbeing, you'll be helping individuals to live as independently as possible.

We pride ourselves on ensuring our staff feel valued, supported and encouraged to develop their skills. As well as our Support Worker induction, you can expect continual opportunities to learn and grow, as you work within our wider multi-disciplinary teams.

The service operates 8am to 8pm, 7 days a week, and you can expect a range of shift patterns, including up to two weekends per month. Please mention your preferred location in your application. A full driving license and access to a vehicle is required for this role.

The Trust is unable to offer Skilled Worker visa sponsorship for this post and applicants must have the right to work in the UK.

### Main duties of the job

As a Community Support Worker, you will:

* Deliver delegated clinical, rehabilitation, and personal care in patients' homes

* Undertake observations and support assessments

* Support people with frailty, long-term conditions, mobility needs, and end-of-life care

* Work as a lone worker, with robust clinical support

* Accurately record care using electronic systems

What We're Looking For

* NVQ Level 3 (or equivalent experience) or strong care experience

* Experience in a care, hospital, or community setting

* Ability to work independently and manage a varied workload

* Compassionate, reliable, and good communication skills
