We are actively recruiting Healthcare Community Support Workers to join our Urgent Response and Hospital at Home service, with vacancies across Eastleigh, Southampton, and Lymington.

The service runs from 8am to 8pm and we ask for a maximum of 2 weekends a month.

Our Urgent Response and Hospital at Home service provides rapid, short-term care in patients' own homes, helping to avoid unnecessary hospital admissions, support earlier discharge from hospital, and manage acute episodes safely in the community.

You will work as part of a friendly, experienced multidisciplinary team including nurses, therapists, paramedics, and doctors.

This is an excellent opportunity if you are:

* An experienced HCA, support worker, or carer

* Looking to move into an NHS community role

* Seeking a role with variety, autonomy and opportunities for learning and development

### Main duties of the job

As a Community Support Worker, you will:

* Deliver delegated clinical, rehabilitation, and personal care in patients' homes

* Undertake observations and support assessments

* Support people with frailty, long-term conditions, mobility needs, and end-of-life care

* Work as a lone worker, with robust clinical support

* Accurately record care using electronic systems

What We're Looking For

* NVQ Level 3 (or equivalent experience) or strong care experience

* Experience in a care, hospital, or community setting

* Ability to work independently and manage a varied workload

* Compassionate, reliable, and good communication skills
