The Discharge Unit is seeking an enthusiastic, highly motivated Health Care Assistant to join their team.

The successful candidate will work as part of a hard working team who focus on supporting the safe, timely and effective discharges from hospital care to home, rehab services or community-based care.

We aim to ensure a positive experience for all patients and families who access our service and hope to improve patient flow and reduce unnecessary hospital stays.

### Main duties of the job

The role of the Health Care Support Worker in the Discharge Unit is to provide support with care needs in  both the short and long stay areas.

The aim is to provide safe, compassionate and person-centred care that supports patients in becoming ready for discharge and transitioning safely home or another care setting.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
