The teams consist of Health Visitors, Community Staff Nurses, Community Nursery Nurses and Health Visitor Assistance under the management of a team Lead. We are proud to be a community that cares, delivering our services to our clients as a team, whilst supporting our employers as individuals ### Main duties of the job The post holder will be a qualified Health Visitor. You will work autonomously as part of the Health Visiting Team managing a designated caseload of children and families, in order to improve their health and enable them to reach their full potential. You will manage an identified enhanced caseload and work collaboratively with other agencies to provide high quality care to children and families. Child protection work will involve gathering and analysing information to make informed assessments, appropriate referrals and participating and contributing at case conferences and core group meetings. To identify and monitor the health needs of a given population and facilitate the implementation of evidence based health care. To develop public health initiatives in line with local and national priorities which promote, protect and improve the health of the population and reduce health inequalities.
