Family Help / Multi-Agency Hub / MACPT – Families First Partnership Programme (FFPP) 2-Year Fixed Term Funded Post

We are seeking an experienced and motivated Health Navigator with safeguarding expertise to join our integrated safeguarding and family support system in Hounslow, working across Family Help, the Multi-Agency Hub, and MACPT pathways alongside health, social care, education, police, and wider partners.

This post is part of the Families First Partnership Programme (FFPP) — a national transformation programme focused on earlier help for families, stronger multi-agency collaboration, and reducing escalation into specialist statutory services. This is a 2-year fixed term, externally funded post supporting system-wide transformation in early help and safeguarding practice.

You will play a key role in ensuring that health and safeguarding information is identified, interpreted, and used effectively to inform multi-agency decision-making. Acting as a key link between health services and safeguarding partners, you will support the early identification of risk and need, and contribute to timely, coordinated responses for children and families.

What We’re Looking For: Experienced safeguarding practitioner with a health or care background Strong experience of safeguarding children and multi-agency working Ability to interpret and communicate complex health and safeguarding information Understanding of safeguarding thresholds, risk, and statutory processes Experience working across health, social care, or early help services Confidence in multi-agency decision-making environments Strong organisation and ability to manage competing priorities Commitment to early help, prevention, and reducing inequalities

## Person Specification

### Knowledge & Experience

**Essential**

- Experience of undertaking learning reviews and implementing changes in practice
- Knowledge of developing guidelines, policy, and procedures
- Knowledge of multi-agency and multi professional working
- Excellent interpersonal, oral, and written communication skills
- Ability to present information effectively in a format easily understood
- Able to collaborate with senior managers and clinicians

### Education/ Qualifications

**Essential**

- Registered Nurse
- Educated to degree level
- Extensive experience working with children in a variety of health settings (inclusive of community)
- Evidence of continued professional development with relevant further qualifications
- Undertaken Safeguarding Supervision course and experience of delivering supervision (1:1 / group)
- Involvement in planning, facilitating, and delivery of training packages
- Experience of staff management and leadership (able to provide clinical and managerial supervision)
- l expertise and awareness of the health needs of children at risk of harm

**Desirable**

- MASH / Social Care experience
- Educated or working towards a master’s level of practice
